Back to back deep Class II restorations, with tooth #5/1.4 at serious risk of needing endodontics, however, as the tooth tested vital, we will watch this over time and treat as required. A back-to-back sectional matrix set-up was completed on #4, with an “intentional knife-edged overhang” created using firm cervical wedging from the WaveWedge in an attempt to establish a contact on the distal and an injectable approach.
Sadly, the patient had incipient-becomes-active caries on the lingual aspect of #4 wrapping to the distoproximal root surface that was only identified on post-op radiograph. Caries is like a forest fire, surprising us with its rate of progression in just 6 months. This will be treated with a direct lingual approach next visit.
