This lovely patient had Lumineers completed overseas and presented in this challenging state. After initial restorative stability was gained relative to caries and periodontal stability ensured, the patient completed a course of clear aligner therapy with the goals of better supporting the buccal corridors in the smile. DSD was completed and a 3D anatomic wax-up completed with the view of an aligned, pleasant, yet character-rich composition.
The APT technique as described by Gurel was employed to control facial reduction, however, underneath the existing restorations revealed a structure-compromised situation. Bonded cores were completed and provisionals fabricated via the shrink-to-fit technique with GC TempSmart, characterized by Optiglaze Color. The patient very quickly broke off #4-6 with the rest remaining intact in situ until the delivery date. The final view is an immediate post-cementation view, replete with cramped papillae which will recover well over the next few weeks.
