Marginal ridge fractures or peripheral rim fractures can happen as a result of occlusal effect caries according to my mentor Graeme Milicich with cyclical compression on the buccal cusp which if extreme enough and even in the absence of amalgam occlusal restorations can lead to “cracks” in the enamel. It happened in this case as the compression dome was interuppted via attrition, exposing dentin to tensile stresses, which when paired with buccal cusp compression compromises the proximal enamel. There is no corresponding crack past the DEJ in dentin, but these defects in enamel can act as superhighways for bacterial ingress, which is what happened in this case here.
The Garrison system was utilized successfully to reachieve a broad, stable contact. The restorative used was a A3.25 Grandio SO shade.
