This lovely 24 year old gentleman presented status post dental trauma to teeth #7-9 with all three teeth requiring endodontic treatment. The fracture extent was significant on tooth #7 with a subgingival fracture margin detectable 1.5mm subgingivally on the palatal aspect.
Instructions to the orthodontist were to extrude the tooth by 1.5mm to allow for restoration of the fracture margin. Extrusion resulted in coronal migration of the zenith, thus necessitating crown lengthening (closed flap transsulcular technique using the Gemini Evo and Kois Bakeman Wedelstaedt chisel) of #7.
A DSD-guided approach revealed the need to restore #10 also to achieve balance better proportions of the incisors.
